Job Summary:

Qualifications Requested:

  • Practice universal precautions when caring for patients
  • Provide incidental household services as are essential to the patient’s well being at home.

JOB SUMMARY:

The occupational therapist provides care to clients in their home accordance with the plan of care and with participation by the family, nurse or other person as necessary. All functions shall be performed in accordance with the established policies and practices of the agency. The occupational therapist reports directly to the nursing supervisor.

QUALIFICATIONS:

  • Prepare clinical and progress notes.
  • Advise, consult and teach appropriate treatment procedures and supportive activities to clients, family members and other agency staff.
  • Establish household management routines.
  • Design and/or order appropriate adaptive/assistive devices and/or splints to promote optimal level of functioning.
  • Record and report to the physician the clients reaction to the therapy program or any changes.
  • Instruct clients and their families in activities of daily living.
  • Instruct the family in the clients total occupational therapy program.
  • Instruct other health team personnel regarding occupational therapy for specific clients.
  • Attend care conferences as deemed necessary.
  • Participate in in-service programs.
  • Serve on the clinical record review committee.
  • Perform other related duties and responsibilities as deemed necessary.

Job Summary:

The Registered Nurse Case Manager, as a member of the interdisciplinary team, is responsible for the provision of skilled nursing assessment, planning and provision of patient care in order to maximize the palliation of symptoms.  In addition, the nurse case manager is responsible for identifying patient or family needs and providing supportive care and appropriate services in accordance with the attending physician’s orders, the interdisciplinary team plan of care, and the policies and procedures of the agency.

Qualifications:

  • Be eighteen (18) years of age or older
  • Minimum associate degree in nursing from an accredited school
  • Possession of a current license to practice in the State of New Mexico as a registered nurse.

Duties and Responsibilities:

  • Assume primary responsibility for patient and family caseload, including the assessment, planning, implementation and evaluation phases of the nursing process.
  • Obtain data on physical, psychological, social and spiritual factors that may impact patient and family’s needs and initiate intervention by other members of the interdisciplinary team.
  • Initiate communication with the attending physician, other interdisciplinary staff members and other agencies for the purpose of coordinating optimal care.
  • Effectively utilize resources to assist in meeting the patient’s needs.
  • Provide regular visits to re-assess the patient’s needs and update the plan of care.
  • Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician.
  • Perform clinical pronouncement of death and provide support to family
