Who Is Appropriate for Hospice Care?

Eligibility Criteria
Patients must meet the following eligibility criteria to be appropriate for hospice care:
- Patients with a prognosis of six months or less if the disease runs its normal course
- Hospice can continue after six months as long as the patient continues to meet eligibility criteria
- Prognosis is certified by a primary care or community physician and hospice physician
- Patient and family desire palliative care as opposed to curative treatment
Hospice Services Include:
- Nursing visits by experienced RNs who are on call 24 hours a day
- Home visits by our physicians when medically indicated
- Personal care by nursing assistants
- Short term inpatient care
- Respite care
- Medications related to the hospice diagnosis
- Medical equipment and supplies
- Visits from social workers and chaplains who can provide spiritual support and counseling as well as assistance with community resources
- 13 month bereavement program
